Independent Personal Services form

The Independent Personal Services Form must be attached to the Honorarium or Expense voucher when making payments for services of $600.00 or less per voucher or $2,000.00 or less per year. This form must be attached for payments of any amount to speakers and entertainers.

Payments for larger amounts will require the attachment of a Professional Services Agreement form instead of the Independent Personal Services form except for payments to speakers and entertainers.

Payments to non-resident aliens will require the attachment of the IRS Form 8233.

The Independent Personal Services form was revised in February 2000. Please download the revised version from the ISU Forms Website and discontinue using the old one.
