Facilities Planning and Management (FP&M) is a service organization providing the university community with a safe, comfortable, and usable physical environment. The term "facilities" refers to all the buildings, grounds, walks, drives, parking lots, and utility systems belonging to the University. FP&M consists of the following divisions: Business Services, Facilities Planning, Facilities Services, Support Services, and Utilities Services. FP&M must authorize all major organized outdoor activities occurring on campus. This includes entertainment, speakers, or food service. Policy Statement
Classroom Scheduling manages the university's general classrooms and auditoriums, assigning rooms to meet the needs of all class sections. In addition, the staff assists all special event organizers scheduling general classrooms and auditoriums each year. Classroom scheduling is also responsible for the maintenance and improvement of classrooms to meet the environmental and technological expectations of students and faculty.
Special events include, but are not limited to, departmental meetings, seminars, oral defenses, course review sessions, and much more. Student Organizations wanting to reserve a classroom/auditorium for a group meeting need to submit a room request form. This form is available via the FP&M homepage at http://www.fpm.iastate.edu/roomscheduling/department_form. Please call 294-5338 for details on how to reserve a classroom, auditorium, or with questions regarding group meetings.

Chemistry Stores, located on the northwest corner of Gilman Hall, primarily handles the bulk storage of chemicals, laboratory apparatus, glassware and gas cylinders used in research and teaching for the university. All departments are encouraged to purchase their supply items from Central Stores. Sales to individuals are not authorized. Purchases may be made by filling out a Central Stores requisition (call 294-8484 for a copy) or by terminal entry where available. Requisitions may also be sent by fax to 294-6394. Merchandise may be picked up at the warehouse or Central Stores' personnel will deliver it at no cost. (2001)
Central Stores provides a service in the acquisition of commonly used materials to academic departments and Facilities Planning and Management. Electrical, hardware, plumbing, paint, household, lumber, and paper items are in stock. The Central Stores' warehouse is located in the General Services building. Maintenance supplies and materials that can be purchased in quantity and at a savings in cost will be stocked at Central Stores.
Other services provided by Central Stores include sales of respirators, safety shoes, and safety glasses. Trucking services are also provided. Trucks, equipment, and personnel will transport materials or equipment from place to place. A furniture assembly service is also available by calling Central Stores. Rental storage space is available at both the S.E. 5th Street warehouse and CACC locations.

The University Book Store (UBS), located in the Memorial Union orders and stocks all required textbooks (new and used) and course materials for class use. In addition, the Book Store maintains a large selection of books for reference and general reading. Books not in stock can usually be special ordered. A wide selection of office, school, art, photography, and engineering supplies, as well as computer software and electronics and photo finishing are also available. Office supplies not stocked are available by special order and on-line. UBS offers a selection of greeting cards and gift items and is the official retail outlet for ISU insignia clothing and merchandise. Departments generally receive a special discount on most purchases except for special orders, textbooks, computer software, and electronics. Purchases can be made using cash, personal check, debit cards, credit cards, University and federal purchasing cards, and intramural forms. In addition, faculty and staff in an appointed and/or budgeted position may apply for a personal charge account. Call 294-0243 for information on personal charges.

UBS reserves the option to charge the department for shipping charges for any title dropped after classes begin if returnable, full charge for the books if not returnable, for any books bought back from students upon adoption and subsequently dropped, or for any unsold courseworks copies beyond 5% of printing quantities if not used in subsequent semesters.
Reason for the policy
The University Book Store is a self-supporting department of Iowa State University. When materials are ordered upon request of a department, there is a sharing in the cost of procuring and producing materials for classes. While the University Book Store rarely exercises this policy and absorbs thousands of dollars in expense, if a department abuses this accommodation, the store reserves the right to charge the department. Because course material information is shared with the public, it is assumed that the information being shared is as accurate and timely as possible.

Cash register receipts are required for any textbook refund. Refunds are allowed up to two weeks following the first day of classes in the fall and spring semesters for all full session classes. New texts will be in "new" condition for a refund. Worn new texts may be refunded as "used" with approval of the textbook department supervisor. Refunds are granted within five days after class begins for all full summer session classes. All short, early start, and late start classes have a refund period of 24 hours from time of purchase during the first three days of class. No refunds will be given after this time frame. Refunds may be given for packages that have an intact package ISBN and the package/bundle can be reshrunk. No refunds will be given for opened packages if: (1) the package cannot be reshrunk; (2) tapes or software are part of the package; (3) the ISBN for package is missing. There are no refunds for CourseWorks.

It is the responsibility of the University Book Store to assure the availability of course materials and to protect the assets of the University. The refund policy assures book accepted for returns were sold at the store, and also covers the expense of operations. Books and merchandise (excluding electronics and computer software) in new condition may be refunded within 30 days of purchase date, provided it is in salable condition and there has been no abuse of refund privilege. Cash refunds will be made for cash, check, and debit card purchases. Purchases made by credit card will be credited to the same card used to make the purchase. Clothing received as a gift, for which there is no receipt, may be exchanged for a different size. Returns without a receipt may be exchanged for like value or in-store credit for the current retail value of the item being returned. No refunds will be allowed on books with software if the disk sleeve has been opened unless disk is defective. After the 30-day period following purchase date, refunds will be given for defective merchandise at the store's discretion. No refunds will be allowed on test preps, study guides, job and career guides, computer books, or clearance merchandise. Special order merchandise must be in original packaging, in salable condition, and made within two weeks of receipt. Departments should retain purchase order number and order date for return authorization.

The Biosafety program provides oversight and coordination for the safe use of human and animal pathogens, biological toxins, human blood, and recombinant DNA. This involves monitoring laboratory safety practices, performing annual biological safety cabinet evaluations, providing information on the safe disposal of biohazardous waste, and advising researchers regarding pertinent regulatory guidelines and required permits. Biosafety program personnel administer the activities of the Iowa State University Biohazards and Public Health Committee, facilitate employee participation in the Iowa State University Occupational Medicine Program, and serve on the Iowa State University Committee on Animal Care.

Prevention of fires in the workplace is the responsibility of all workers, students, and visitors to Iowa State facilities. All employees should be familiar with and understand:
The Fire Prevention Plan covers important aspects of fire safety and includes sections on ignition sources, identifying fire hazards, and proper control procedures. Departmental supervisors should review with employees information covered in the Fire Prevention Plan and cover any departmental procedures that may vary from the plan.
Periodically, inspectors from the State Fire Marshal's Office conduct fire safety audits of campus and residential buildings for compliance with state building codes. Environmental Health and Safety (EH&S) coordinates the correction of cited deficiencies and performs follow up inspections. ISU employees are required to cooperate with Fire Marshal inspections and comply with fire safety and building code requirements.
EH&S oversees fire safety and prevention programs at ISU, which include identification of fire hazards, enforcing fire prevention rules, emergency planning, and fire/incident investigations. All fires and fire hazards should be reported to EH&S.

Policy Statement
All construction activities in Iowa State buildings affecting life safety systems, exit ways, mechanical systems, laboratories, storage areas, processes involving hazardous materials, and activities that could damage asbestos containing materials and lead paint should be forwarded to Environmental Health and Safety (EH&S) for review. EH&S works with project designers and engineers to assure that construction activities and designs comply with current codes, standards, and regulations. (2001)
All projects forwarded to EH&S are reviewed for compliance with current state building codes and other national standards. The plan/process is usually completed within two weeks and comments returned to the person submitting the plan or process. Bid issue plans for capital projects are required to be submitted to the State Fire Marshal for final approval.
Reason for the policy
EH&S is able to ensure that safety requirements are met during the design process and incorporated into construction projects. It is more cost-effective to incorporate safety measures during a project's design phase than the construction phase or after construction has been completed.

Policy Statement
The role of the Occupational Health group is to protect the health and well being of employees and the public from chemical, microbiological and physical health hazards present at or emanating from the workplace. These issues may be present as a result of workplace activities, ineffective building systems or environmental influences. Industrial hygienists are available to investigate workplace problems and then initiate corrective actions. The Occupational Health section provides comprehensive services including work site evaluations, monitoring, training and consultation, indoor air quality investigations, asbestos and lead project management, chemical exposure monitoring, ergonomic work site reviews, laboratory safety surveys, lighting surveys, noise level monitoring, ventilation review, and a material safety data sheet (MSDS) library. Information on occupational health services and access to MSDSs online is available at the Environmental Health and Safety (EH&S) web site.
Reason for the policy
Federal and state agency regulations that apply to workplace health and safety dictate certain university actions. Relevant regulatory authorities include the Occupational Safety and Health Administration (OSHA), U.S. Department of Agriculture (USDA), U.S. Department of Transportation (DOT), U.S. Environmental Protection Agency (EPA), Iowa Department of Public Health (IDPH), Iowa Department of Natural Resources (IDNR), etc.

Policy Statement
Training programs have been developed to meet the safety, health, and regulatory needs of the faculty, staff and students of Iowa State University. Many work procedures and activities conducted by members of the university community have regulatory requirements for information and training. When training is required for a work procedure, it must be provided to employees before they begin performing that duty. Currently, Environmental Health and Safety (EH&S) offers courses on nearly 40 different online and classroom safety training topics and has over 70 titles in a video library available for checkout. Training courses are open to the Iowa State University community.
To ensure that all employees are able to receive the training they need, EH&S offers the Online Training Center (http://www.ehs.iastate.edu/OTC.htm). Through the Center, users are able to create and maintain an electronic personal training profile. Once a profile has been created, users can:

Training programs support the Department of Environmental Health and Safety's mission of providing and ensuring a safe and healthful environment for employees, students, and the visiting public. Many of the EH&S's training programs also meet regulatory requirements.
